General comments
Voriconazol doseren op geleide van spiegels
Voriconazol opladen: iv 6 mg/kg 2dd gedurende dag 1
Anidulafungin opladen: iv 200 mg 1dd, gedurende dag 1
Alternatief voor voriconazol is isavuconazol, iv 200mg 3dd gedurende 2 dagen, gevolgd door isavuconazol iv 200mg 1dd.
Voor hematologiepatiënten wordt in principe dubbeltherapie gegeven.
Indien een patiënt NIET aan een van de onderstaande criteria voldoet kan worden volstaan met voriconazol monotherapie
